Define Your Visit Goals
Prior to your initial consultation, take time to reflect on the main reason for your visit and what you hope to accomplish. Whether you are managing a long-term condition, recovering from an injury, or seeking treatment for chronic discomfort, having a clear objective will help us develop the most effective plan for your care. Our patient intake questionnaire is available here Patient Intake Form. If you are unable to print this, we will provide you with one to complete prior to your visit.

Describe Your Symptoms
A thorough understanding of your symptoms and medical history allows us to provide the best possible care. Please consider the following questions before your visit:
- When did your symptoms start? Was there a specific injury (a single incident that immediately led to your symptoms), or did they develop gradually over time?
- Do your symptoms occur constantly or only with certain activities?
- If you experience pain, where is it located, and how severe is it on a scale from 0 to 10 (0 being none and 10 being the worst pain imaginable)?
- Have your symptoms improved, worsened, or remained the same over the past few months?
- What activities (work, sports, hobbies) are important to you? How have your symptoms impacted your ability to participate?

Compile Your Treatment History
As a specialized referral practice, we understand many of our patients have received prior treatment. Please bring a complete record of your past treatments specific to your orthopedic reason for seeking care, including:
- Medications you have used for your condition
- Any injections (type, location and date)
- Physical therapy history (duration and type of therapy)
- If you have had previous surgery, please bring a copy of your operative report and/or arthroscopic images from your previous surgeon’s office.

Obtain Any Prior Imaging
If you have had imaging done on your affected joint, please contact the facility where it was performed and request both the report and actual images on a CD. This includes X-rays, CT (CAT) scans, and MRI scans. If you have not had any recent X-rays, we may request new imaging on the day of your visit.
